Free Web Event: A Close Look at Care Coordination Within Patient-Centered Medical Homes - West Virginia's Experience


Who should coordinate care? How can care coordination be ensured for populations who transition frequently? Join us for the second web event in this series hosted by the AHRQ Health Care Innovations Exchange to learn about West Virginia’s innovative Medical Home and Transitions Initiatives.
