The Centers for Medicare and Medicaid Services has put out a Q&A document for state Medicaid directors that covers various topics on the maintenance-of-effort provisions in the ACA.

July 22, 2011 the U.S. Department of Health and Human Services approved Kentucky's request to adjust the 80 percent MLR standard.

The Department of Health and Human Services (HHS) released draft rules on the implementation of Consumer Operated and Oriented Plan (CO-OP) program under the Affordable Care Act.  This program will fund the creation of CO-OPs to offer qualified health plans in insurance exchanges.

This proposed rule would implement standards for states related to reinsurance and risk adjustment, and for health insurance issuers related to reinsurance, risk corridors, and risk adjustment consistent with title I of the Patient Protection and Affordable Care Act (ACA).
